What Is Shockwave Therapy?
How It Works
Shockwave therapy – formally known as extracorporeal shockwave therapy or ESWT – uses high-energy acoustic sound waves delivered through the skin to the affected tissue. At Manly Cove Podiatry we use the Chattanooga Intelect RPW 2, which delivers radial pressure waves that spread through the treatment area and stimulate the body’s natural healing response.
The mechanism is well understood. The pressure waves create a controlled microtrauma in the tissue – disrupting degenerative tissue, stimulating blood flow, triggering the release of growth factors, and kickstarting a genuine healing response. For conditions like plantar fasciitis and Achilles tendinopathy, where the tissue has entered a chronic degenerative state that the body has essentially stopped trying to repair, this mechanical stimulus is often what is needed to restart the healing process.

Shockwave therapy is not a first-line treatment – it is best suited to conditions that have become chronic or have not responded adequately to conservative management. The conditions we most commonly treat with shockwave therapy at Manly Cove Podiatry are:
- Plantar fasciitis: particularly cases that have been present for more than 3 months and have not fully resolved with stretching, orthotics, and load management. Shockwave therapy for plantar fasciitis has strong evidence and consistently good results
- Achilles tendinopathy: both midportion and insertional presentations where progressive loading programs have plateaued. Shockwave therapy for Achilles tendinopathy is one of the most evidence-backed applications of the treatment
- Shin splints: chronic medial tibial stress syndrome that has not settled with the standard load management and strengthening approach
- Plantar fascia tears and chronic heel pain: where tissue remodelling is needed beyond what rest and exercise can achieve
- Other tendinopathies: including peroneal tendinopathy and tibialis posterior tendinopathy where conservative treatment has been exhausted
One important note – shockwave therapy is contraindicated in patients under 18 due to open growth plates, in those with certain blood disorders or on anticoagulant medication, and over areas of active infection or malignancy. We assess suitability at your appointment before recommending it.
What to Expect at a Shockwave Therapy Session in Manly
The Assessment First
Before recommending shockwave therapy, I carry out a full biomechanical assessment to confirm the diagnosis, identify any contributing mechanical factors, and determine whether shockwave is the right next step for your specific presentation. There is no point applying shockwave to a tissue if the mechanical overload driving the problem has not been addressed – the two need to go together.
The Session Itself
Each shockwave therapy session at Manly Cove Podiatry takes approximately 15 to 20 minutes. A gel is applied to the skin over the treatment area to help conduct the waves. The handheld applicator is then moved over the affected area, delivering the pressure waves. Most patients describe the sensation as a deep aching or tapping pressure – it is not painless, but it is well tolerated. The intensity is adjustable and we work within your comfort level.
You will typically be weight-bearing and mobile immediately after. Some patients experience a temporary increase in soreness for 24 to 48 hours following the first session – this is a normal part of the healing response being stimulated, not a sign that things are getting worse.
How Many Sessions Do You Need?
Most patients require 3 to 6 sessions of shockwave therapy, spaced one week apart. The majority see meaningful improvement within that range. Chronic cases that have been present for a year or more may need the full 6 sessions to achieve the best result. We reassess after each session and adjust the plan accordingly – there is no fixed protocol applied regardless of how you are responding.
Does Shockwave Therapy Hurt?
Honest Answer
Most patients find it uncomfortable rather than painful – there is a difference. The sensation is described variously as a deep aching, a tapping pressure, or a temporary sharp feeling directly over the most affected area. The intensity varies depending on the tissue being treated and the pressure settings used. We start conservatively and increase based on your response and tolerance.
The discomfort during the session typically settles within minutes of finishing. As mentioned, some patients experience a temporary flare in symptoms for one to two days after the first session. By the second or third session, this post-treatment soreness usually reduces significantly as the tissue begins to respond.
Is Shockwave Therapy Right for You?
When It Is Worth Considering
Shockwave therapy is worth considering if you have been dealing with a persistent foot or lower limb condition for more than 3 months, have tried conservative treatments including stretching, loading exercises, orthotics, and footwear changes without achieving full resolution, and are looking for a non-surgical option to accelerate tissue healing.
It is not a substitute for addressing the underlying mechanics driving your condition. The best outcomes come from combining shockwave therapy with the correct loading program and any biomechanical corrections identified at assessment. On its own, without addressing contributing factors, the results are less reliable.
